Transfer Information

Please gather the following pieces of information, as they will be required to complete the transfer process:

1. Copy of registration or last recertification audit report and all surveillance reports subsequently issued.
2. Copy of current valid certificate.
3. Provide evidence that the prior registrar either accepted or closed the minor non-conformances from their last audit. If not, then corrective action taken for each is to be provided for review.
4. Provide evidence that major non-conformances issued over the current cycle have been resolved. If any major non-conformances were issued on the last audit, evidence must be provided of an on-site special audit.
5. Provide trending of complaints since the last audit and action taken to effectively deal with any negative trend.
